Dr. Miller is an enrolled provider for the following insurance companies:

Blue Cross / Blue Shield
Medicaid
Health Choice
Cigna

Many insurance plans provide out-of-network benefits, even when Dr. Miller is not an enrolled provider.  Each prospective clients is encouraged to contact their insurance company to ask about their mental health benefits, including when the benefit year is, any deductible, co-pay, pre-certification required, number of visits per year allowed, or other relevant coverage details.

Dr. Miller’s administrative assistant, Maira Durham, files all insurance claims as a service to Dr. Miller’s clients.  Co-pays or  deductible amounts are expected to be paid to Dr. Miller at the time of service.

A $70 fee is charged if a client does not show for an appointment or fails to give 24 hour notice for a cancelled appointment.
